PERMISSION AND UNDERSTANDING
APPENDIX A: FERPA RIGHTS
The District will not administer or distribute to students any survey or other instrument for the purposes of collecting or compiling personal information for marketing or selling such information, with the exception of the collection, disclosure, or use of personal information collected for the exclusive purpose of developing, evaluating, or providing, educational products/services for, or to, students or educational institutions.
Pursuant to federal laws, the District will seek parental consent prior to and will not request, nor disclose, the identity of a student who completes any federally funded survey or evaluation (created by any person or entity, including the District) containing one or more of the following:
- Political affiliations.
- Mental and psychological problems.
- Illegal, antisocial, self-incriminating, and demeaning behavior.
- Criticism of other individuals with whom the student or the student's family has a close family relationship.
- Relationships privileged under law, such as relationships with lawyers, physicians, and ministers.
- Religious practices, affiliations, or beliefs of students or the student's parent/guardian.
- Income, except when the information will be used to determine a student's eligibility to participate in a special program or to receive financial assistance under such a program.
